We facilitate community coalitions that are focused on addressing key health priorities at the individual, family, environmental, and policy level with the goal of improving the health and wellbeing of our residents. We do this in in partnership with community organizations and interested citizens.
Our health priorities are congruent in Washington and Ozaukee County. While the priorities are the same, we understand that demographics, populations, and services are different in each county. We will work to address those discrepancies in the design of our work within coalitions.
Public Health 3.0
Public Health 3.0 emphazises collaborative engagement and actions that directly affect the social determinants of health inequity. It refers to a new era of enhanced and broadened public health practices that goes beyond traditional public health department functions and programs.
Community Health Improvement Plan (CHIP)
This health assessment is a workplan that is completed every five years. The CHIP process starts with a series of community conversations focused on understanding health needs. Key community thought leaders help us prioritize by defining goals, objectives and strategies for the next operational cycle. Coalitions take the defined goals and objectives and develop programming and interventions.
Community Health Needs Assessment (CHNA)
This health assessment is conducted in partnership with area hospital systems. It guides strategic goals and planning for these organizations, as well as helps inform the CHIP report by providing quantitative and qualitative data.
